Fill out the "Camper Application" or "CIT Application" Part 1 (page numbers 1 and 2).
Mail the application with your deposit check to:

Fill out the "Camper Application" or "CIT Application" Part 2 (page numbers 3 and 4) and sign the "Parental Agreement," "Release Form" and have your camper sign the "Camper's Contract of Understanding."
Fill out the "Emergency Contact and Health History Form"
Have your medical practioner complete and sign the "Medical Exam Form"
Mail the completed "Parent and Camper Agreements," "Emergency Contact and Health History Form" and "Medical Exam Form" to the camp office at the address listed above (#2). All forms should be mailed to the camp office at least 2 weeks before the camp session starts. We must have all forms, completed, before your camper may leave for camp.
